Eccentric Step Test

The Eccentric Step test is used to assess the integrity of the patellofemoral joint. This test is also called the step-down test.

To perform this test, ask the patient to step down from an elevated position with the weight borne by the involved limb.

This test is considered positive if it reproduces the patient’s anterior knee pain with or without the patient reporting a crackling or a grating sensation. This test is considered to be one of the most accurate tests for patellofemoral pain syndrome.
