External Rotation Recurvatum Test

The external Rotation Recurvatum test is used to detect the presence of posterolateral rotary instability of the knee.

With the patient in the supine position, grasp their great toes, lift the lower extremities from the table and then compare the alignment of each knee.

The test is considered positive if comparison reveals a hyperextended knee on the involved side together with external rotation of the tibia, creating the appearance of a genu varum deformity.

This indicates injury to the posterior cruciate ligament or the anterior cruciate ligament or both. The positive results also indicate the involvement of the posterolateral corner structures, which include:

• Arcuate-popliteus complex
• Lateral collateral ligament
• Biceps femoris tendon
• Posterolateral capsule
• Iliotibial band
